Annual Permit Renewal Checklist
The documents Filipino business owners need to file every January. Print, check off, sleep better.
Barangay Clearance
On or before January 20- Latest barangay clearance (previous year)
- Official receipt of latest payment
- Brgy. business permit form (filled out)
- Valid ID of owner / authorized representative
Mayor's / Business Permit (LGU)
On or before January 20- Latest barangay clearance (current renewal)
- Latest Mayor's Permit
- BIR Form 0605 (annual registration fee, ₱500)
- Latest community tax certificate (Cedula)
- Audited Financial Statements (or Sworn Declaration of Gross Sales)
- Sanitary Permit (current renewal)
- Fire Safety Inspection Certificate
- Lease contract / proof of property ownership
BIR Annual Registration
On or before January 31- BIR Form 0605, ₱500 annual registration fee
- Updated COR (BIR Form 2303), confirm RDO and tax types
- Books of Accounts re-stamping (if applicable)
- Authority to Print receipts / invoices (validity check)
Sanitary Permit
Annually, varies by LGU- Application form (LGU sanitary office)
- Health certificates of food handlers (where applicable)
- Latest Mayor's Permit
- Pest control certificate (food / hospitality)
SSS / PhilHealth / Pag-IBIG (employer)
Ongoing, verify employer status- Confirm SSS R-1A is up to date for new hires / separations
- PhilHealth ER-2 reconciled with active employees
- Pag-IBIG MCRF / contributions remitted via online portal
LGU requirements vary. The list above is for General Trias / Cavite and similar Cavite municipalities. Always verify with your local Business Permits and Licensing Office.
